Important Information: Please ensure the information you are entering is the exact name as it appears on your government issued valid photo identification. Also all other information are entered accurately without any error and following the form format (for example for date of birth – MM/DD/YYYY). This identification is required by you at the test venue and on your licence application upon successful completion of your test. If you have entered the legal name/any other information incorrectly and arrive at the test venue, you will not be able to attempt your test, and will be required to pay the associated fee for your next booking.
Note If you do not have First name or Last name, Please write N/A on the CRLF form for the missing name.
Example 1: if your First name is John Doe and no last name, then write John Doe under first name and N/A under last name. Example 2: if you do not have a first name, last name is John Doe, then write N/A under first name and John Doe under last name.
DO NOT SUBMIT A FIRST AID CPR CERTIFICATE IF: 1. is NOT permanent - means temporary issued for 60/90 days 2. NOT WSIB Approved 3. Photo Identification does NOT match with the name on the certificate 4. is NOT equivalent to St. John Ambulance course Emergency Level First Aid
