Important Information:

HOW TO PROPERLY FILL OUT THIS FORM:

If you started at a site at 6:05am when you were scheduled to start at 6am, then you must select at 6:15am start time in this form. Same way if your shift ended at 18:07pm then you must select 18:15 and do the calculation accordingly. However you can select end time 18:15 or vice versa if you notified the dispatch center on that particular date if your relief was late/emergency. Do not just select extended end time because you were just briefing the other guard as that is providing false information. Every guards must come to work at least 10 minutes before the shift time to get ready and be briefed.

Only use one submission per site. Do not include multiple sites in one form if you worked multiple locations for the same pay period. In that case, use each form for each site.

ANY FALSE INFORMATION PROVIDED ON THIS FORM MAY IMPOSE DISCIPLINARY ACTION INCLUDING DISCHARGE OF THE EMPLOYMENT FROM DARKCITY.
